Go below deck and use the toilet
Every crew discovered this the same way — someone wandered into the cabin and read the prompt out loud. “Revive a player through the toilet. Just so we know, you’ve got to come down to the toilet to revive a player.” It is in the boat’s bathroom, one deck down.

It costs a Golden Fish
Hotfix #4 lists: “Fixed an issue where the revive toilet did not require a Golden Fish to revive a player.” The developer was fixing a build where reviving was free. In the release version it is not — somebody has to have caught one.
This also matches what one crew was doing on camera without knowing why: “See if I can get a goldfish. See if that fixes you.”

Whoever is reviving has to stop what they were doing
The toilet is below deck, the fish is wherever you left it, and you carry one item at a time. A revive pulls a live player off the helm or the harpoon for as long as it takes, which is why a death mid-hazard tends to cascade.

If nobody can revive you
Checkpoints catch the run
Hotfix #4 fixed multiple checkpoint issues and reduced the time needed to restart after the boat is destroyed. Hotfix #3 extended the checkpoint reload delay after a destruction, and 06.09H fixed pause not working after loading a checkpoint. The system exists and has been worked on repeatedly.

And the map will fish you out if you fall through the world
Both patches added teleport recovery volumes beneath the map to recover players who end up outside the playable area. If you clip out of the world, that is a known state the game tries to handle rather than a lost run.
Not confirmed
We do not know whether the Golden Fish you catch and the “goldfish” named elsewhere in the patch notes are the same item — one note fixes a goldfish being too large in storage, another names a Golden Fish for reviving. They read like the same fish, but we are not going to state that as fact until the release build settles it.
What this does to how you play
A revive cost turns fishing from an income activity into insurance. A crew that has spent every fish at the dock has no way to bring anyone back, and a crew that hoards them is not paying the barrier tolls. Yes. The developer confirmed it in Hotfix #4, in a line fixing the opposite bug: 'Fixed an issue where the revive toilet did not require a Golden Fish to revive a player.' So a revive has a real cost, and that cost is a fish you have to catch.
+What happens if you die and nobody revives you?
You wait, or the run resets at a checkpoint. Checkpoints exist and the developer has fixed several checkpoint bugs, and separately reduced the restart time after the boat is destroyed. Dying is not run-ending, it is a tax on everyone still alive.
+Where do you get a Golden Fish?
You fish for it. One creator reported that a hook baited with corn produced a goldfish. That is a single source from one demo build, so treat it as a lead rather than a recipe.
